For EACH new patient please complete BOTH the NHS Scotland GP registration form and our local new patient form.
These forms enable you to share your personal details with us including contact details, your previous GP details, Past Medical History and current medication etc.
Please include your mobile number and email address if possible.
Please consider if you wish to register for online repeat prescriptions as well.

NHS Scotland GP Registration Form
Please complete this form and email it to gram.strathdonclinical@nhs.scot or hand in a copy at the surgery. It is not necessary to sign the form. Electronic submission is sufficient.
